A Modified Novel Technique for Condylar Positioning in Mandibular Bilateral Sagittal Split Osteotomy

•

Unrecognized condylar sag with malpositioned condylar segments in the 3-dimensional (3D) plane will result in unacceptable malocclusion, poor esthetic outcomes, and masticatory and TMJ functional impairment.2

The ideal condylar position in the glenoid fossae is critical for condylar function and for the accuracy and stability of postoperative occlusion and TMJ function.

Proximal ramus position and related condylar CR in orthognathic surgery can depend on several characteristics:

  1. Selection of osteotomy design in relation to advancement or mandibular setback movement
  2. Dislocating contact between proximal and distal bone segments at the osteotomy site
  3. Type of fixation system
  4. Transverse intercondylar variation related to amount of advancement or mandibular setback
  5. Accuracy in condylar CR position at fixation
